Property Insurance Application

  • Note that all names listed on title are required to be listed on the policy. This does NOT include Mortgage or financing parties, who will be added later as an Additional Insured.
  • Note that this form needs to be completed separately for each location.
  • We just need it to match up for your completed application with the file we've already started for you!
  • NOTE: Unless you have other reasons for a different date, it is usually cheapest to change at the renewal.
  • We just need to know a bit more about their operations. E.g. Accountant office, photography studio, retail store, restaurant etc...
  • This includes renters and your own personal use.
  • Note: This refers to post secondary students living on their own. Dependents of tenants don't count.

If you have insurance, you can check your old policy, or ask us to help with this number, but average replacement cost per square foot typically starts around $300-$400+.
  • Note: This is the replacement cost of everything nailed down in the unit, such as upgrades to the flooring or cabinets that you are responsible for. This amount can be zero if this doesn't apply.
  • Please provide a yearly/annual estimate for the next 12 months.
  • Note: This is for everything that isn't nailed down, such as furniture, fridge, stove etc... If the unit is rented completely empty this value can be zero.
  • This is primarily covers claims like slip & falls etc.
